Certificate of Criminal Record
 
 Eligibility for application
 You are eligible for a Certificate of Criminal Record if...
You are registered as a resident or an alien resident of Hokkaido
or   
You live outside Japan and were registered as a resident or an alien resident of Hokkaido before leaving Japan
and   
You can show that you need to present this certificate to a public entity outside Japan
 Depending on the country to which you will be submitting the certificate, we might not be able to accept your application immediately. Please contact us by phone before your visit to confirm the document that we will need from you. (If you do not speak Japanese, please ask a Japanese-speaker to call us.)
 You need to apply for the certificate in person.

Note:  If you are or were registered as a resident or an alien resident of a Japanese prefecture other than Hokkaido, please contact the police headquarters in the relevant prefecture. If you wish to apply for the certificate from Hokkaido and not that other prefecture, please contact us by phone before your visit. (If you do not speak Japanese, please ask a Japanese-speaker to call us.)

 Items required for application
 If you are a registered resident or alien resident:
1.  Valid passport
2.  Official document which proves that the certificate is necessary
3.  I.D. (One of the following)
Driver's license
Basic Resident Register Card (Juki card)
Certificate of Residence issued no more than six months before the application
Certificate of Items Stated in Resident Register issued no more than six months before the application
Alien Registration Certificate
Residence Card
Special Permanent Resident Certificate
Note:  If you do not have any of the above-mentioned documents, please contact us by phone before your visit. (If you do not speak Japanese, please ask a Japanese-speaker to call us.)

 If you live outside Japan and were a registered resident or alien resident of Hokkaido before leaving Japan:
1.  Valid passport
2.  Official document which proves that the certificate is necessary
3.  Any documents that show your current address, such as a driver's license or mail addressed to you
4.  One of the following documents which prove that you were a registered resident or alien resident of Hokkaido
Copy of a Deleted Residence Record (Japanese: "Johyo") (issued no more than six months before the day of application)
Family Register Label (Japanese: "Fuhyo") (issued no more than six months before the day of application)
Note:  If you are unable to present any of the above-mentioned documents, please contact us by phone before your visit. (If you do not speak Japanese, please ask a Japanese-speaker to call us.)

 No fee is necessary for this procedure.

 Days and times for application
   From 8:45 a.m. to 5 p.m. on weekdays, except from December 29 to January 3.

 Issuance of the certificate
   It takes about 10 days for the certificate to be issued.
 We do not mail the certificate.
 Only the applicant or a proxy who has a letter of attorney can receive the certificate.

 Other
   If you live outside Japan, you can apply for the certificate at a Japanese consulate or at the Japanese embassy. However, it takes about three months for the certificate to be issued.
